Each year, all students (new and returning) are required to complete certain forms electronically through Operoo.com.  Additional forms specific to each grade are listed below and will be posted to this site as soon as the 2021 versions are finalized.

Kindergarten

1ST, 2ND, 3RD AND 4TH GRADES

  • Young Adult Literature Access Permission
  • Locker Contract

6TH GRADE

  • Community Service Information & Consent
  • Grade 6 Off-Campus Lunch Procedures & Permission (begins Spring semester, but only if a permission slip is returned!)
  • Locker Contract
  • Young Adult Literature Access Permission
  • Wellness Room Waiver Form
